Specialist Nurse for Transition to Adult Services

The Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust is seeking an experienced and dedicated Specialist Nurse to lead the Transition service for children with complex needs moving into adult services. As a key member of our team, you will be responsible for providing direct specialist care, education, and support to patients and their families during this critical phase of their care.
Main Responsibilities- Provide hospital-based care for the majority of the time, working closely with multidisciplinary teams to ensure seamless transitions for patients.
- Develop and implement individualized care plans to meet the unique needs of each patient, in accordance with national and Trust policies.
- Conduct needs-based assessments of facilities and services to identify areas for improvement and develop strategies to enhance patient care.
- Support young people aged 16 and above when placed on adult wards, providing guidance and education to facilitate a smooth transition.
- Collaborate with ward managers and specialist nurses to create a safe and supportive environment for patients, staff, and families.
- Stay up-to-date with the transition agenda and best practices, sharing knowledge and expertise with colleagues to drive quality improvement.
