Specialist Nurse for Transition to Adult Services

About the Role
The Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust is seeking an experienced and skilled nurse to lead the transition service for children with complex needs moving into adult services. As a Transition Nurse Specialist, you will be responsible for providing direct specialist care, education, and support to patients and their families during this critical phase.
Key Responsibilities
- To provide high-quality, individualized care to patients and relatives/carers without supervision in collaboration with the multi-professional team.
- To work as an autonomous practitioner, taking responsibility for the ongoing clinical management of patients in the clinic and ward/department settings and into the community through liaison with their local healthcare providers.
- To act as an advocate for patients and their families, providing expert support and advice based on best practice and close communication with other members of the multi-professional team.
- To facilitate high-quality care and support to patients and their families, promoting the Trust's values and high-performance standards.
Requirements
- Registered Children Nurse Qualification
- Previous hospital nursing post, working with children and young people
- Experience working in a transitional role for children moving into adult services
- Up-to-date training in Safeguarding children and young people
