We are seeking an experienced and skilled nurse to lead our Transition Service for children with complex needs moving into adult services. As a Specialist Nurse for Transition to Adult Services, you will be responsible for providing direct specialist care, education, and support to patients and their families during this critical phase.
Key Responsibilities- Provide clinical-led transitional care to patients with complex musculoskeletal, spinal, and oncology conditions.
- Work collaboratively with the multi-professional team to facilitate high-quality, individualized care to patients and relatives/carers.
- Act as an advocate for families, providing expert support and advice based on best practice and close communication with other team members.
- Develop and implement training programs to educate staff on transitional care best practices.
- Contribute to the development of policies and procedures related to transitional care.
- Registered Children Nurse Qualification.
- Previous hospital nursing experience working with children and young people.
- Experience working in a transitional role for children moving into adult services.
- Up-to-date training in Safeguarding children and young people.
