The Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust is seeking an experienced Specialist Nurse to lead the Transition service for children with complex needs moving into adult services. This is an exciting opportunity for a skilled nurse to make a real difference in the lives of patients and their families.
Key Responsibilities
- To provide direct specialist care, education, and support for patients with transitional care needs.
- To act as a specialist resource when liaising with and facilitating the best use of all available services.
- To undertake a needs-based assessment of the current facilities and services available to patients and their families during transition.
- To develop the service to assist patients during their transition phase.
- To provide independent and unsupervised support to patients, families, and carers during the transition period.
