The Robert Jones and Agnes Hunt Orthopaedic Hospital NHS Foundation Trust is seeking an enthusiastic individual to lead the Transition service for children with complex needs moving into adult services.
Main Duties- To provide direct specialist care, education, and support for patients with transitional care, acting as a specialist resource when liaising with and facilitating the best use of all available services.
- To undertake a needs-based assessment of the current facilities, services available to patients and their families during transition and to continue to develop the service to assist patients during their transition phase.
- To provide care for transitional care patients, in accordance with national and Trust policies and Frameworks.
- To be flexible in the approach to the clinical area and provide clinical support as required.
- To act as an advocate for those families giving expert support and advice based on best practice and close communication with other members of the multi-professional team.
