Key Responsibilities- Provide expert nursing care to patients in their own homes, assessing their needs and developing individualized care plans.
- Work collaboratively with our multidisciplinary team, including consultants, GPs, pharmacists, therapists, and healthcare support workers, to ensure comprehensive care.
- Contribute to the development and implementation of clinical policies and procedures, ensuring best practice and evidence-based care.
- Participate in clinical supervision and leadership, supporting the growth and development of our team.
- Level 1 Adult Nurse Registration with the Nursing and Midwifery Council (NMC).
- Degree or alternative post-registration qualifications in a relevant discipline or portfolio of significant relevant experience supplemented by diploma specialist training, experience, short courses.
- Health Assessment and Examination skills within clinical practice or willingness to work towards.
- IT skills, data collection & input, audit and evaluation.
- Knowledge of current community services and utilize research within nursing to maintain best practice.
