About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our Urgent Community Response & Virtual Wards service in Shropshire, Telford & Wrekin. As a key member of our team, you will play a vital role in providing high-quality care to patients in their own homes, working closely with our multidisciplinary team to deliver a seamless transition through services.

Key Responsibilities
  • Provide expert nursing care to patients in their own homes, assessing their needs and developing individualized care plans.
  • Work collaboratively with our multidisciplinary team, including consultants, GPs, pharmacists, therapists, and healthcare support workers, to ensure comprehensive care.
  • Contribute to the development and implementation of clinical policies and procedures, ensuring best practice and evidence-based care.
  • Participate in clinical supervision and leadership, supporting the growth and development of our team.
Requirements
  • Level 1 Adult Nurse Registration with the Nursing and Midwifery Council (NMC).
  • Degree or alternative post-registration qualifications in a relevant discipline or portfolio of significant relevant experience supplemented by diploma specialist training, experience, short courses.
  • Health Assessment and Examination skills within clinical practice or willingness to work towards.
  • IT skills, data collection & input, audit and evaluation.
  • Knowledge of current community services and utilize research within nursing to maintain best practice.
