Job Overview">

We are seeking a highly skilled Mental Health Practitioner to join our multidisciplinary team at Cambridgeshire & Peterborough NHS Foundation Trust. The ideal candidate will have experience working in a community setting and possess excellent leadership skills.


About the Role">

This role involves providing holistic psychosocial assessments, developing individual packages of care, and implementing brief interventions for patients experiencing acute mental health crises. You will work collaboratively with our team to deliver dynamic solution-focused care and promote recovery models that empower patients and carers.


Responsibilities">
  • Screen referrals, prioritize effectively, and provide holistic assessments
  • Develop and implement individual packages of care, including risk management and crisis intervention
  • Collaborate with multidisciplinary teams to ensure timely, effective, and integrated care delivery
  • Promote patient-centered care, empowering patients and carers through education and support
  • Liaise effectively with referrers, healthcare professionals, and other stakeholders

Requirements">
  • Registered Nurse, Occupational Therapist, or Social Worker with current registration
  • Experience working in a community setting, with leadership roles and staff management
  • Excellent communication, organizational, and time management skills
  • Ability to contribute to service development, manage change, and work under pressure
  • Able to cover shifts over a 24-hour period, 7 days a week
