This is an exciting opportunity to work in a dynamic and innovative environment, contributing to the delivery of high-quality care to individuals with mental health needs.
About the RoleThis position involves working closely with Primary Care teams, facilitating mental health assessments and supporting GPs with joint decision-making regarding secondary care referrals, signposting, and transitioning service users from secondary mental health services back to Primary Care. Your expertise will inform GP management plans for those with mental health needs in primary care.
Key Responsibilities- To support service users with transitioning from secondary care into primary care.
- To be a valued core member of primary care teams within an LCG, using specialist mental health knowledge.
- To develop links and foster a culture of partnership between Primary Care Teams and CPFT Adult Mental Health Services.
- To promote the use of treatment protocols and care pathways leading to increased integration of service users within Primary Care setting.
We are looking for a registered mental health professional with significant post-qualifying experience in the relevant care group. The ideal candidate will have excellent negotiation and conflict resolution skills, as well as the ability to assess and interpret service user conditions with appropriate actions.
BenefitsWe offer a competitive salary of £40,000 - £60,000 per annum, depending on experience. As a member of our team, you will also receive access to ongoing training and development opportunities, as well as a comprehensive benefits package.
