Clinical Nurse Specialist Leader

This is an excellent opportunity for a Clinical Nurse Specialist to join our team and make a real difference in the lives of patients and their families. As a key member of our Community Specialist Palliative Care Team, you will be responsible for providing high-quality patient care, working closely with other healthcare professionals to achieve the best possible outcomes.
Main Duties- To provide clinical expertise and leadership within the community setting, focusing on symptom management and quality of life for patients with life-limiting illnesses.
- To work collaboratively with multidisciplinary teams to develop and implement effective care plans that meet the individual needs of patients.
You will need to have a strong understanding of palliative care principles and practices, as well as excellent communication and interpersonal skills.
