Lead Support Worker Role
At Home Group, we're passionate about empowering our customers to live their best lives. As a Lead Support Worker, you'll play a vital role in supporting individuals with complex mental health needs to develop the skills and confidence to live independently.
Key Responsibilities:
- Create personalized support plans that promote independence and well-being.
- Collaborate with a small team of Support Workers to deliver high-quality care and support.
- Carry out risk assessments, support planning, goal setting, and regular reviews to ensure customer needs are met.
- Build strong relationships with customers, families, and colleagues to achieve shared goals.
About the Role:
- Permanent, part-time (17.5 hours per week) position.
- Two shifts per week on a rota basis.
- Able to use technology for creating and updating support plans, completing online learning, and collaborating with colleagues.
- Enhanced DBS check required, which will be paid for by Home Group.
