Spiritual Care Practitioner

Full-time

Spiritual Care Practitioner at St. Joseph's Hospice of London. We serve our residents, clients, and caregivers in our 10-bed residence and in the community. We also have an agreement to provide spiritual support to the London Middlesex Palliative Care Outreach Team.

Spiritual Care Practitioner Role

The Spiritual Care Practitioner supports a variety of programs and services designed to promote a person-centred, seamless approach to care, supporting the Psychological, Spiritual, Social, Practical, End of Life Care/Death Management and Loss and Grief Domains of Care of individuals and their loved ones throughout the Palliative Care - Chronic Disease Continuum, including bereavement.

SPIRITUAL CARE and COUNSELLING:

  • Through spiritual care assessments identifies, guides and facilitates the spiritual support and/or interventions for the unique faith needs of individuals in our Residential and Community Programs and the London-Middlesex Palliative Care Outreach Teams (PCOTs), and their identified circle of care.
  • Provides spiritual care to individuals and their loved ones across the palliative care continuum.

PROGRAM IMPLEMENTATION and EVALUATION:

  • In collaboration with members of the interdisciplinary team, provides input into the direction of spiritual and supportive care programming to meet the diverse needs of those we serve.

Education and Qualifications

  • University degree in Religious, Theological or Pastoral studies
  • Master's degree in counselling, Religious, Theological or Pastoral studies
  • Certificate in Pastoral Care (Canadian Association for Pastoral Practice), or equivalent
  • Member of Canadian Association for Spiritual Care (CASC) is preferred
  • Certificate in the Fundamentals of Hospice Palliative Care, or the commitment to obtain
  • Willingness to work flexible hours and possess reliable personal transportation and be approved by insurance carrier to drive personal vehicle
