Home Care Specialist Job Opportunity

We are seeking a skilled Home Care Specialist to join our Aylesbury Adult Community Healthcare Team. This role involves providing nursing interventions to patients in their home environment.

The successful candidate will be responsible for contributing to processes for service and quality improvement, assessing physical and psycho-social needs of patients, and delivering therapeutic interventions based on best available evidence.

Main Responsibilities:

  • To contribute to service and quality improvement processes.
  • To assess physical and psycho-social needs of patients and deliver therapeutic interventions.
  • To manage own case load and deliver service delivery to patients with various medical conditions.

Requirements:

Salary: £37,000 - £42,000 per annum depending on experience.
