Support Worker for Vulnerable Adults

About the Role
In this challenging yet rewarding position, you will work collaboratively with patients to identify their individual needs and preferences. Your excellent communication skills and ability to build rapport with patients will enable you to deliver personalized care and support.
Responsibilities
- Assess patient needs and develop personalized care plans
- Deliver high-quality care and support to patients
- Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes
Requirements
- NVQ Level 2 or equivalent qualification in Health and Social Care (desirable)
- Minimum 6 months healthcare experience in the UK
- Evidence of full immunity to Hepatitis B, MMR, Tuberculosis & Varicella
- Right to work in Northern Ireland without restriction
