Job Title: Community Care Professional Chesterfield S40

We are seeking a compassionate and skilled Community Care Professional to join our team in Chesterfield, covering the S40 postcode and within a 10-mile radius.

This is an exciting opportunity to provide personal care and support to individuals in their own homes, making a positive impact on their lives.

Key Responsibilities:
  • Provide personal care and support to individuals in their own homes
  • Assist with activities of daily living, such as bathing, dressing, and grooming
  • Administer medications as directed by healthcare professionals
  • Assist with meal planning and preparation
  • Perform light housekeeping tasks, including laundry and cleaning
  • Accompany clients to appointments and outings
  • Monitor and report any changes in clients' health or behavior
  • Maintain accurate and up-to-date documentation of care provided
