Job Summary

We are seeking a skilled and compassionate Community Care Specialist to join our team in Chesterfield S40. As a Community Care Specialist, you will provide personalized care and support to individuals in their own homes, promoting their independence and well-being.

Key Responsibilities
  • Provide personal care and support to individuals in their own homes, including assistance with activities of daily living such as bathing, dressing, and grooming.
  • Administer medications as directed by healthcare professionals.
  • Assist with meal planning and preparation.
  • Perform light housekeeping tasks, including laundry and cleaning.
  • Accompany clients to appointments and outings.
  • Monitor and report any changes in clients' health or behavior.
  • Maintain accurate and up-to-date documentation of care provided.
Requirements

To be successful in this role, you will need:

  • A Full UK Driving License and access to a vehicle.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including weekends.
  • Basic knowledge of health and social care principles.
