About the Role

We are seeking a skilled and compassionate Community Care Professional to join our team in Chesterfield, covering the S40 postcode and within a 10-mile radius.

As a Community Care Professional, you will provide personalized care and support to individuals in their own homes, assisting with activities of daily living, administering medications, and accompanying clients to appointments and outings.

Key Responsibilities
  • Provide personal care and support to individuals in their own homes
  • Assist with activities of daily living, such as bathing, dressing, and grooming
  • Administer medications as directed by healthcare professionals
  • Assist with meal planning and preparation
  • Perform light housekeeping tasks, including laundry and cleaning
  • Accompany clients to appointments and outings
  • Monitor and report any changes in clients' health or behavior
  • Maintain accurate and up-to-date documentation of care provided
