We are seeking a skilled and compassionate Community Care Professional to join our team in Chesterfield S40. As a key member of our team, you will provide high-quality care and support to individuals in their own homes, promoting their independence and well-being.
Key Responsibilities- Provide personal care and support to individuals, including assistance with activities of daily living such as bathing, dressing, and grooming.
- Administer medications as directed by healthcare professionals.
- Assist with meal planning and preparation.
- Perform light housekeeping tasks, including laundry and cleaning.
- Accompany clients to appointments and outings.
- Monitor and report any changes in clients' health or behavior.
- Maintain accurate and up-to-date documentation of care provided.
