About the Role

We are seeking a skilled and compassionate Community Care Professional to join our team in Chesterfield, covering the S40 postcode and within a 10-mile radius.

As a Community Care Professional, you will provide personalized care and support to individuals in their own homes, assisting with activities of daily living, administering medications, and accompanying clients to appointments and outings.

You will also be responsible for monitoring and reporting any changes in clients' health or behavior, maintaining accurate and up-to-date documentation of care provided, and participating in on-call duties.

Our ideal candidate will possess a Full UK Driving License, have access to a vehicle, and be able to work every other weekend. We offer a competitive hourly rate, paid training, and opportunities for career progression.
