About the Role

We are seeking a highly motivated individual with recent experience in support work, risk management, and caseload management. You will work closely with colleagues to develop and deliver co-produced, bespoke customer support and move-on plans, ensuring a consistent asset-based support approach for all customers.

Your expertise in building effective relationships with customers and stakeholders will be invaluable in shaping the service and driving positive outcomes. You will also complete regular reviews of support and risk assessments for customers, identifying and promoting opportunities for them to access relevant external support services.

Key Responsibilities
  • Co-produce bespoke customer support and move-on plans
