About the Role
We are seeking a highly skilled and experienced Allied Health Professional (AHP) Duty Coordinator to join our Urgent Community Response team. The successful candidate will play a vital role in supporting Therapy teams to triage and prioritise referrals, completing wellbeing calls and support with urgent therapy visits for assessment, equipment provision and support rehabilitation of patients with Physiotherapy or Occupational Therapy needs.
Main Responsibilities
- Lead the triaging of referrals coming into the Therapy teams.
- Support the co-ordination of patient management by allocating visits and managing effective on/off boarding processes.
- Work flexibly and visit people in their own homes, conducting basic assessments and treatment plans, and supporting therapists with their patient caseload.

Person Specification
- Good level of general education including A levels and GCSE or equivalents including functional skills level 2.
- An advanced / higher level apprenticeship for example AP apprenticeship or similar and / or NVQ level 4 in relevant subject or equivalent level of knowledge and experience.
- Able to demonstrate substantial experience in health and care settings including in the relevant field of work.
- Experience of computerised record systems.
- Evidence of interprofessional working.
