About the Role

We are seeking a highly skilled Allied Health Professional (AHP) Duty Coordinator to join our Urgent Community Response team. The successful candidate will work closely with Therapy teams to triage and prioritize referrals, complete wellbeing calls, and support with urgent therapy visits. This is an exciting opportunity to develop services that support patients to remain at home where possible.

Main Responsibilities

The post holder will have previous experience in a role based in clinical settings and be part of a highly supportive team that uses an inter-professional model to support a broad range of patients. Key responsibilities include:

  • Working as part of a team and confident in working autonomously
  • Leading the triaging of referrals coming into the Therapy teams
  • Supporting the co-ordination of patient management by allocating visits and managing effective on/off boarding processes
  • Visiting people in their own homes, conducting basic assessments and treatment plans, and supporting therapists with their patient caseload
