Perinatal Outreach Specialist

We are seeking an experienced and dynamic individual to join our team as a Perinatal Outreach Specialist.
As a key member of our Specialist Perinatal Outreach Team, you will provide outreach services across Greater Manchester for individuals in the perinatal period.
Your primary responsibility will be to work closely with the ward and community Multi-Disciplinary Team (MDT) to identify patients who would benefit from working with the outreach team. You will also perform home visits, manage and triage referrals, and act as bed manager for Andersen Ward Mother and Baby Unit.
The successful candidate will need to be flexible and able to work a shift pattern that meets the needs of the population.
The estimated salary for this role is £42,000 - £60,000 per annum, depending on experience.
