Job summary

GMMH are looking to recruit an experienced and dynamic individual into the role of perinatal outreach practitioner for an exciting new team working to bridge the gap between inpatients and community perinatal services.

The Outreach Practitioner will be a member of the Specialist Perinatal Outreach Team, providing outreach services across the wider Greater Manchester area for those in the perinatal period.

The Outreach Practitioner will have a personal case load and will be responsible for assessment, formulation and prescribing of care planning including discharge planning within agreed protocols. They will be providing support to complex cases and ensuring that both mother/birthing person and baby receive the best care. Providing the opportunity to support patients in the community who are vulnerable to admission to Andersen ward mother and baby unit (MBU) and be able to facilitate early and safe discharge.

The post holder will have the knowledge of or a passion for perinatal mental health, and will advise the multi-disciplinary team on expert, safe and effective models of care and treatment.

The post holder will effectively require working across the range of services as determined by service user need.

The successful practitioner is required to be qualified as either a registered nurse, occupational therapist or social worker and be registered with the relevant governing body.

Main duties of the job

The Practitioner will work closely with the ward and community MDT to identify patients who would befit from working with the outreach team, perform home visits at times on a lone working basis, manage and triage referrals into the ward and the outreach team, act in the role of bed management for Andersen ward mother and baby unit. The post would ideally suit someone with experience of working with families during the perinatal period or a passion for working, learning and developing in this area.

The outreach team also facilitate safe discharge from the ward, developing robust crisis and contingency plans to support leave and eventual discharge from the ward. Once discharged from the ward, the teamwork with mothers/birthing people and their families to offer ongoing support.

The outreach team will also support mothers/birthing people in the community who are vulnerable to admission to the MBU providing short term interventions alongside the patients allocated care coordinator within the wider specialist perinatal community service.

The successful practitioner will need to be flexible within the period of Monday to Friday 9am until 7pm, with a shift pattern to meet the needs of the population.

The successful practitioner is required to be qualified as either a registered nurse, occupational therapist or social worker and be registered with the relevant governing body.
