Perinatal Outreach Practitioner

We are seeking an experienced and passionate individual to join our team as a Perinatal Outreach Practitioner.

As a member of our Specialist Perinatal Outreach Team, you will provide outreach services across Greater Manchester for individuals in the perinatal period. Your primary responsibility will be to work closely with the ward and community MDT to identify patients who would benefit from working with the outreach team.

Key duties include:

  • Assessing and formulating care plans for patients
  • Managing referrals and triaging cases
  • Acting as bed manager for Andersen Ward Mother and Baby Unit

The estimated salary for this role is £48,000 - £70,000 per annum, depending on experience.
