Job OverviewWe are seeking an experienced and dynamic individual to join our Perinatal Outreach Practitioner team at Greater Manchester Mental Health NHS Foundation Trust.The successful candidate will work closely with ward and community multidisciplinary teams to provide outreach services, assessment, and case management for vulnerable mothers and babies.
