Job Description

This is an exciting opportunity to join our team as a Community Perinatal Care Coordinator.

You will work closely with the outreach team to facilitate safe discharge from hospital, developing robust crisis and contingency plans to support patients. You will also provide short-term interventions alongside the patient's allocated care coordinator within the specialist perinatal community service.

Key duties include:

  • Working with patients in their homes to assess and formulate care plans
  • Providing support to complex cases and ensuring mother and baby receive the best care
  • Facilitating early and safe discharge from hospital

The estimated salary for this role is £45,000 - £65,000 per annum, depending on experience.
