We are seeking a dedicated Patient Care Specialist to join our team at Wallingford Community Hospital.

About the Role

This is an exciting opportunity for a qualified RN with a passion for delivering high-quality patient care. As a Patient Care Specialist, you will be responsible for providing comprehensive care to patients with a wide range of conditions, including rehabilitation and complex discharge management.

You will work collaboratively with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient. Your excellent communication and interpersonal skills will enable you to build strong relationships with patients, families, and other healthcare professionals.

A key aspect of this role is your ability to remain calm under pressure and adapt to changing situations. You will work effectively in a fast-paced environment, prioritizing tasks and managing your time efficiently to ensure seamless delivery of care.

The ideal candidate will have a strong therapeutic presence, empathy, and compassion. You will be able to communicate effectively with patients who have varying levels of health literacy and cognitive function.

The salary for this position is approximately £30,000 - £42,000 per annum, depending on experience. We offer a competitive benefits package, including annual leave, pension scheme, and access to professional development opportunities.
