About the Job
As an experienced Healthcare Assistant, you will have the opportunity to make a real difference in the lives of people living with life-limiting conditions. This is a challenging and rewarding role that requires empathy, effective communication skills, and a passion for delivering high-quality care.

This position involves providing support to patients in their own homes, working as part of a multidisciplinary team to promote choice in the delivery of care and support. You will also be responsible for observing service users' conditions and reporting changes, participating in care planning, and supporting service users to have their views and experiences considered in the way the service is provided and delivered.

The ideal candidate will have a minimum of 1 year's experience of working in a UK-based health or social care role and have a clear understanding of palliative, end of life, and community care. They will also have a strong commitment to delivering exceptional patient care and a willingness to develop their skills and knowledge in specialist palliative care.
