Learning Disability Care Coordinator

About the Job: As a Learning Disability Care Coordinator at Enfields, you will play a vital role in supporting individuals with learning disabilities to live fulfilling lives. Your responsibilities will include conducting comprehensive assessments, developing care plans, and managing caseloads.
Requirements:
- A recognised Social Work Qualification.
- Understanding of relevant legislation.
- Commitment to continual professional development.
Key Tasks:
- Identify the needs, strengths, and preferences of adults with learning disabilities through comprehensive assessments.
- Develop and implement individualised care and support plans in collaboration with service users, their families, and other professionals.
- Manage a caseload of adults with learning disabilities, ensuring regular reviews and adjustments to care plans as necessary.
Working Arrangements: Full-time, permanent contract.
Salary Range: £40,000 - £60,000 per annum.
