Clinical Lead for Chronic Disease Management

About the Role
Hall Green Health is seeking an experienced and skilled Enhanced Practice Nurse to join our team as a Clinical Lead for Chronic Disease Management. The successful candidate will have a specialist interest in Diabetic Medicine and will be responsible for leading the management of chronic diseases, including diabetes, within our practice.
Key Responsibilities
- Lead the development and implementation of chronic disease management plans
- Collaborate with the nursing management team to review and update clinical policies and procedures
- Provide clinical leadership and support to the nursing team in the management of chronic diseases
- Work closely with the clinical director to ensure the delivery of high-quality patient care
Person Specification
- Relevant HCP qualification (RGN/HCPC)
- Masters in advanced practice, or working towards
- Independent Prescriber
- Specialist in Diabetic Medicine
