Role Overview
Tripod Partners is seeking a highly skilled Senior Social Worker to join our Continuing Healthcare team. As a key member of our team, you will work in partnership with Cambridgeshire and Peterborough Integrated Care Board to determine individual eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process.
Key Responsibilities
- Lead on practice and performance development through high-quality supervision and support of staff in line with The Care Act 2014 principles.
- Complete Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments, and Best Interest Decisions (where applicable) to identify individual needs and support Multidisciplinary teams' recommendations on eligibility for NHS Continuing Healthcare.
- Implement care and support for individuals not eligible for NHS Continuing Healthcare but eligible for care and support from Cambridgeshire County Council in line with the Care Act 2014.
Requirements
- Qualified Social Worker with Adults CHC experience.
- Ability to work in partnership with Cambridgeshire and Peterborough Integrated Care Board.
- Hybrid working required, with two days per week in the office at Scott House, 5 George Street, Huntingdon.
