Adult Care Specialist

We are seeking a skilled Adult Care Specialist to join our South Locality Team (Adults) in High Wycombe. As an Advanced Practitioner, you will play a vital role in delivering goal-based support for individuals to achieve greater independence.
Key Responsibilities:
- Work in accordance with the Better Lives framework to promote equality and fairness in all aspects of care delivery.
- Provide personalized information and advice, safeguarding, assessment, support planning, and review to individuals and their carers.
- Take account of people's risks and safety at all times, identifying mitigations to reduce that risk.
- Support people to achieve the outcomes that are most important to them using all available resources and taking responsibility for the public purse.
