Job Summary

This role involves working as part of our Outpatient team, providing expert nursing care to patients with various health needs. As a Nurse Care Specialist, you will be responsible for assessing patients' conditions, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities

  • Assess patients' physical and emotional needs, identifying areas requiring attention and developing targeted care plans
  • Deliver high-quality nursing care, adhering to hospital policies and procedures
  • Work collaboratively with multidisciplinary teams to ensure comprehensive patient care
  • Maintain accurate records of patient interactions, treatments, and outcomes
  • Participate in quality improvement initiatives, contributing to the development of best practices and protocols

Requirements

  • Current NMC registration
  • Proven experience working in a similar role or relevant healthcare setting
  • Excellent communication and interpersonal skills, with ability to build strong relationships with patients and colleagues
  • Able to work effectively in a fast-paced environment, prioritizing tasks and managing workload efficiently
  • Commitment to ongoing professional development and continuous learning
