About the Role

We are seeking a skilled Care Coordination Specialist to join our team in Coastal West Sussex. The successful candidate will play a vital role in ensuring that patients receive compassionate and high-quality end-of-life care.

The primary responsibility of this position is to act as a key point of contact for referrals to the End of Life Care Hub (ECHO). Using excellent verbal and listening skills, the post holder will collect accurate referral information and respond to all referrals in a safe and timely manner.
