About the Role

We are seeking an experienced Advanced Nurse Practitioner to lead our occupational therapy service in care homes and the community. As a key member of our team, you will be responsible for ensuring high standards and quality of care are delivered to service users and carers.

Key Responsibilities
  • Manage a caseload of service users with long-term conditions, terminal diagnosis, and those requiring rehabilitation post-hospital admission.
  • Respect every person's preferred wish while providing nursing care within their own home.
  • Provide the highest quality of care by listening to and acting on feedback.
  • Take personal responsibility for sustainably improving services by implementing change positively and contributing to MDT discussions.
  • Support and mentor colleagues to achieve common goals and objectives.
  • Strive to provide excellent, innovative services that enable individuals to live their lives to the full.
