We are seeking a Recovery Oriented Mental Health Practitioner to join our Transformation Unit in Sheffield. This is an exciting opportunity for a qualified mental health nurse or social worker to work in a dynamic and supportive environment, delivering high-quality care and support to individuals with complex mental health needs.

About the Role:

In this role, you will be responsible for developing and implementing individualized plans of care, using a recovery-oriented approach to promote patient empowerment and self-management. You will work closely with patients to identify their strengths and goals, and develop strategies to support them in achieving their aspirations.

Main Tasks:
  • Conduct comprehensive assessments of new referrals, identifying patient strengths and needs
  • Develop and implement individualized plans of care, using a recovery-oriented approach
  • Provide education and training to patients and colleagues
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Contribute to quality improvement initiatives
Remuneration Package:

This is a full-time position offering a salary of £45,000 per annum, plus a range of benefits including a contributory pension scheme and access to ongoing training and development opportunities.
