About the Job:
Sanctuary Personnel Ltd is seeking an experienced support practitioner to join their team in Parkside, Worcestershire.
This permanent position involves working with adults who have complex needs, requiring intensive support. The ideal candidate will use a strength-based approach to reduce and manage risks, promoting independence and autonomy.
- The successful candidate will collaborate with other professionals to develop a multi-agency approach, enabling adults and their carers to live fulfilling lives within their community.
- Representing the service, adults, and carers' views at various meetings is essential, exchanging information, making decisions, and reviewing progress regularly.
- Excellent relationships with other agencies and the local community are crucial for signposting when necessary, ensuring access to relevant support services.
- The role requires identifying potential deprivation of liberty and ensuring the least restrictive means of care provision prior to legal proceedings, upholding human rights and dignity.
