About the Role

We are seeking a skilled and compassionate Community Healthcare Support Practitioner to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our Early Intervention Service, you will play a vital role in providing high-quality care and treatment to individuals with First Episode Psychosis.

Key Responsibilities
  1. Assess and develop individualized care plans for a defined caseload of patients, ensuring comprehensive and coordinated care.
  2. Act as a Care Coordinator, implementing and adhering to Trust policies on Integrated CPA and Risk Assessment.
  3. Promote the needs of carers, complete carer's assessments, and support local Carer/User Groups.
  4. Administer medication, adhering to nursing responsibilities under the Medicines Act (1971) and local care of Drugs policy.
  5. Participate in the Team Duty Worker rota and adhere to Trust requirements.
  6. Ensure compliance with approved Trust clinical nursing policies/procedures and the Trust's supervision policy.

Person Specification

We are looking for a registered Mental Health Nurse, OT, or Social Worker with relevant experience working with clients with psychosis in Adult and/or CAMHs services. You will possess excellent communication and interpersonal skills, with the ability to assess, plan, implement, and evaluate programs of care.
