About the Role

Dorset HealthCare University NHS Foundation Trust is seeking a skilled and dedicated Community Mental Health Nurse to join our team. As a Community Mental Health Nurse, you will be responsible for providing high-quality care to adults with mental health needs in a community setting.

Key Responsibilities

  • Assess, plan, implement, and evaluate programs of care for patients with mental health needs.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care.
  • Develop and maintain effective relationships with patients, families, and carers.
  • Contribute to the development and implementation of care plans that promote recovery and well-being.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements

  • Registered Mental Nurse with valid registration with the NMC.
  • Experience of working in a mental health environment, with a focus on community care.
  • Ability to assess, plan, implement, and evaluate programs of care.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
