Developmental Older Adult Community Mental Health Nurse

We are seeking a highly motivated and experienced Community Mental Health Nurse/Practitioner to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our community mental health team, you will be responsible for providing high-quality care and support to adults with mental health needs.
Main Duties and Responsibilities- Provide comprehensive and accessible mental health services to adults and their families within a defined locality.
- Work closely with the team and Service Manager to implement the Dorset Healthcare Mental Health Strategy.
- Contribute to effective multi-disciplinary working within the community mental health service and build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality.
- Continually develop clinical, team, and service evaluation and delivery of service.
