Developmental Older Adult Community Mental Health Nurse Specialist

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Dorset HealthCare University NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to adults with mental health needs, working closely with patients, families, and other healthcare professionals to promote recovery and well-being.
Key Responsibilities- Assess, plan, implement, and evaluate programmes of care for patients with mental health needs.
- Conduct risk assessments and develop strategies to manage risk.
- Work collaboratively with multidisciplinary teams to provide comprehensive care.
- Develop and maintain effective relationships with patients, families, and other healthcare professionals.
- Contribute to the development and implementation of care plans, ensuring they are patient-centered and tailored to individual needs.
- Registered Mental Nurse with valid registration with the NMC.
- Experience of working in a mental health environment, with a focus on community-based care.
- Ability to assess, plan, implement, and evaluate programmes of care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
- Ability to prioritize and manage workload, with a focus on delivering high-quality care.
