About the Role

We are seeking a highly motivated Advanced Healthcare Practitioner to join our team at NHS. The successful candidate will have a minimum of 3 years' experience in an acute or primary care setting and possess excellent clinical knowledge in acute and chronic disease management.

The ideal candidate will be able to demonstrate clinical competence in minor illness management, excellent communication and consultation skills, and the ability to work independently and effectively within a team.

This is a challenging yet rewarding role that requires the ability to adapt to change within a working situation. The successful candidate will be expected to promote improving the health of the individual through health promotion, health education, screening, and therapeutic communication.

Responsibilities:

  • To assess and manage patients with undifferentiated presentations
  • To provide high-quality patient care and ensure adherence to local and national guidelines
  • To work independently and effectively within a team
  • To promote improving the health of the individual through health promotion, health education, screening, and therapeutic communication

Requirements:

  • A minimum of 3 years' experience in an acute or primary care setting
  • Excellent clinical knowledge in acute and chronic disease management
  • Ability to communicate effectively with patients and colleagues
  • Experience working within primary or urgent care

Salary: £45,000 - £60,000 per annum
