About the Role:

We are seeking a highly skilled and dedicated Social Worker to join our Mental Health Team at Hampshire County Council. As a key member of our team, you will be responsible for providing high-quality support and services to individuals and their families, promoting independent and safe living within the community.

Key Responsibilities:

  • Collaborate with individuals, their families, providers, and other healthcare professionals to develop and implement personalized care plans.
  • Use your excellent relationship management skills to build trust and rapport with service users, their families, and other stakeholders.
  • Work in a strengths-based way, focusing on empowering individuals to take control of their lives and make informed decisions about their care.
  • Conduct thorough assessments and risk assessments to identify the needs of service users and develop effective interventions.
  • Develop and maintain strong relationships with local community resources, such as libraries and other organizations, to provide access to support and services.

Requirements:

  • Qualified Social Worker with a strong understanding of mental health and community care principles.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and other stakeholders.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple cases simultaneously.
  • Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
  • Current driver's license and access to a reliable vehicle, with the ability to travel to various locations within Hampshire and the Isle of Wight.
