About the Role

We are seeking an experienced and skilled Community Nurse to join our Havant Community Care Team at Southern Health NHS Foundation Trust.

As a Community Health Specialist, you will be responsible for providing high-quality care to patients with complex needs in their own homes. You will work closely with the team to manage a defined caseload, ensuring that referrals are handled according to clinical need and that care plans are recorded in line with Trust and professional standards.

The ideal candidate will have excellent clinical, managerial, and communication skills, with the ability to work autonomously and as part of a team. You will be responsible for coordinating and delegating work, ensuring that care is delivered in a timely and effective manner.

Main Responsibilities
  • Manage a defined community caseload, providing care to patients with complex needs in their own homes.
  • Work closely with the team to ensure that referrals are handled according to clinical need and that care plans are recorded in line with Trust and professional standards.
  • Coordinate and delegate work to ensure that care is delivered in a timely and effective manner.
  • Develop and implement care plans that meet the individual needs of patients.
  • Work collaboratively with other healthcare professionals to ensure that patients receive comprehensive care.
