About the Role
Southern Health NHS Foundation Trust is seeking a skilled professional to join our team as a Clinical Care Navigator. This exciting opportunity will enable you to make a meaningful contribution to the transformation of frontline mental health services.
Key Responsibilities
- To work across a range of clinical and non-clinical services to support the delivery of transforming mental health care.
- To support the planning, co-ordination and transfer of service users across the care pathways.
- To support clinicians to identify care needs and develop effective care plans.
- To assist and identify areas where barriers to health and social care exist and work collaboratively to resolve these.
- To work in a manner which is helpful to the service user, based on principles of recovery, personalisation and choice.
- To develop the role of Care Navigator in order to embed the No Wrong Door approach within the system.
- To work with stakeholders and care teams to enable access to relevant interventions / support and self-management as part of the Care Navigator role.

Person Specification
- Essential qualifications include computer skills, in-depth experience of working in a mental health environment, and ability to fulfill travel requirements of the post.
