Job Title: Occupational Therapist - Adult Social Care Specialist
Job Summary:
We are seeking a highly skilled and experienced Occupational Therapist to join our team at Tripod Partners. As an Occupational Therapist - Adult Social Care Specialist, you will play a vital role in delivering person-centered care to adults, their carers, and families within the community.
Key Responsibilities:
- Assessment and Intervention: Deliver holistic and strengths-based care to adults, including safeguarding, complex health needs, learning disabilities, and more.
- Collaboration and Communication: Work collaboratively with a multidisciplinary team to provide proportionate, personalized, safe, and sustainable responses.
- Equipment and Adaptation Solutions: Utilize knowledge and experience of equipment, adaptation solutions, and Disabled Facilities Grant (DFG) provisions.
- Inter-Agency Collaboration: Engage with inter-agency and multidisciplinary approaches across the council and other statutory bodies.
Requirements:
- Qualifications and Experience: Be HCPC registered with experience in a Local Authority setting.
- Knowledge and Skills: Demonstrate knowledge of person-centered, holistic, and strengths-based practices, as well as experience assessing daily living activities and understanding the impact of physical, learning disabilities, and mental health conditions on occupational performance.
- Personal Qualities: Possess strong communication and relationship-building skills with professionals and partner agencies, excellent time management skills, personal resilience, and a positive approach to risk-taking.
- Professional Development: Be a reflective practitioner committed to ongoing professional development.
