Key Responsibilities:
- Assessment and Intervention: Conduct comprehensive assessments of individuals' daily living activities and develop personalized plans to promote independence and well-being.
- Collaboration and Communication: Work closely with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure seamless care coordination and effective communication.
- Equipment and Adaptation Solutions: Utilize knowledge and experience of equipment, adaptation solutions, and Disabled Facilities Grant (DFG) provisions to support individuals in maintaining their independence.
- Inter-Agency Collaboration: Engage with inter-agency and multidisciplinary approaches across the council and other statutory bodies to ensure proportionate, personalized, safe, and sustainable responses.
