Occupational Therapist

We are seeking an experienced and dedicated Occupational Therapist to join our team in South Derbyshire and Dales. As a key member of our Community Mental Health Team, you will work closely with patients, families, and carers to provide high-quality care and support.
About the RoleThis is an exciting opportunity for a Band 5 Occupational Therapist to work in a dynamic and supportive environment. You will be responsible for assessing patients' needs, developing and implementing occupational therapy plans, and collaborating with other healthcare professionals to ensure comprehensive care.
Main Duties- Conduct occupational assessments and develop individualized plans to promote independence and well-being
- Work closely with patients, families, and carers to understand their needs and preferences
- Collaborate with other healthcare professionals to ensure comprehensive care and support
- Participate in multidisciplinary team meetings to discuss patient progress and develop care plans
- Contribute to the development and implementation of occupational therapy services and policies
To be successful in this role, you will need:
- HCPC registration as an Occupational Therapist
- Degree or diploma in Occupational Therapy
- Experience working with older adults and individuals with dementia and cognitive impairment
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
