Job Summary:

We are seeking a highly skilled and experienced Occupational Therapist to join our team at Tripod Partners. As an Occupational Therapist - Adult Social Care Specialist, you will play a vital role in delivering person-centered care to adults, their carers, and families within the community.


Key Responsibilities:

  • Assessment and Intervention: Conduct comprehensive assessments of individuals' daily living activities and develop personalized plans to promote independence and well-being.
  • Collaboration and Communication: Work closely with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure seamless care coordination and effective communication.
  • Equipment and Adaptation Solutions: Utilize knowledge and experience of equipment, adaptation solutions, and Disabled Facilities Grant (DFG) provisions to support individuals in their homes and communities.
  • Inter-Agency Collaboration: Engage with inter-agency and multidisciplinary approaches across the council and other statutory bodies to ensure proportionate, personalized, safe, and sustainable responses.


Requirements:

  • Qualifications and Experience: HCPC registration and experience in a Local Authority setting, with a strong understanding of person-centered, holistic, and strengths-based practices.
  • Skills and Competencies: Excellent communication and relationship-building skills, with the ability to work effectively with professionals and partner agencies.
