Position Overview

We are seeking a dedicated and skilled Rehabilitation Physiotherapist to join our Community Stroke Services team. This role offers an excellent opportunity to work collaboratively within a multidisciplinary team, focusing on the assessment and rehabilitation of stroke patients.

Key Responsibilities

  • Deliver high-quality physiotherapy services to patients in a specialized area.
  • Function as an accountable and independent practitioner, managing your own caseload effectively.
  • Collaborate with Pathway and Team leads to plan, coordinate, and evaluate the physiotherapy services provided.
  • Support and supervise less experienced physiotherapy staff and students, contributing to their professional development.
  • Maintain accurate patient records using both written documentation and electronic systems.
  • Participate in out-of-hours services as necessary.
  • Continuously enhance your professional skills and competencies.

Clinical Responsibilities

  • Obtain informed consent from patients prior to assessment and treatment, adhering to Trust policies.
  • Conduct comprehensive physiotherapy assessments for patients with diverse and complex needs.
  • Utilize advanced clinical reasoning to diagnose conditions and establish rehabilitation goals.
  • Design and implement individualized treatment plans based on evidence-based practices.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Develop discharge plans and make informed decisions regarding referrals to other services.

Communication Skills

  • Effectively communicate clinical diagnoses and treatment plans to patients and their families.
  • Engage with colleagues and other professionals in a timely and professional manner.
  • Document all patient interactions accurately and in compliance with professional standards.

Professional Development

  • Participate in performance appraisal processes and commit to ongoing professional development.
  • Maintain and enhance specialist knowledge through continuous education and training.
  • Contribute to the training and supervision of junior staff and students.

Management Responsibilities

  • Organize and prioritize your caseload to meet service demands.
  • Mentor less experienced staff and provide guidance as needed.
  • Identify opportunities for service improvement and implement changes in collaboration with senior staff.

Qualifications and Experience

  • Degree in Physiotherapy or equivalent.
  • Membership with the Chartered Society of Physiotherapy.
  • Registration with the Health and Care Professions Council.
  • Demonstrated experience in relevant clinical areas.
